JOB DESCRIPTION  JOB TITLE: CASE MANAGER - OutpatientLOCATION: HURON or MADISONREPORTS TO: CLINICAL SUPERVISOR FLSA  STATUS: NONEXEMPTDEVELOPED: 11/2016  SUMMARYThe job of case manager is a clinical, direct service position.  The job consists of case management services to Community Counseling Services’ clients, most of whom are served through two distinct programs. The first is our CARE program that serves adult individuals identified as having severe and persistent mental illnesses and their families.
